MEMBERSHIP RENEWAL PACK 2008-2009
This pack is for anyone who wishes to renew their membership with the Lifesaving Club for the 2008-2009 season.
Categories include: Nippers and parents, Active Cadets, Active Juniors, Active Seniors, Reserve Actives, Long Service, Life Members, Award Members and Associates.

a) Surf Life Saving Membership Application - please return this form to the Club with payment of your membership fees.
b) Blue card application - All members who are 18 years or older must hold a blue card. Please complete this form and return it to Club administration with your ID to be sighted and photocopied.
c) Blue card renewal form - if your card has expired or is due to expire soon. This link will take you to the Commission for Children, Young People and Child Guardian website. You will need the password the Commission sent you in your reminder notice to access this form. If you do not have a password, please complete a new Blue card application form or contact the Commission for details. Please complete this form and return it to Club administration with your ID to be sighted and photocopied.
d) Approval to confirm a valid Blue card - if you already hold a card but it is not registered with SLSQ.
e) Supporters' Club membership form for lifesaving members who are 18 or older - a new Club membership card will be made up based on this information.
You do not have to pay extra for this membership, it is included in the Lifesaving fee. This form needs to be returned to the Club.
Please allow a couple of weeks for your membership card to be mailed out to you.
f) Medical/consent form - for all members under 18 years of age. A consenting adult must complete this form. Please return it to the club with the appropriate membership application.
Please note that memberships expire on the 31st July and must be renewed before you can participate in any lifesaving activities after this date. This includes surf sports training, patrols and proficiencies.
Fees can be paid by cash, cheque, mastercard, visa or eftpos and must be paid at the Club bar. You can leave your completed forms with the bar staff but please be sure to check that everything is filled in correctly to avoid delays in processing your membership.
